New Patients

 

 

We kindly ask new patients to print and fill out in the New Patient From PRIOR to arriving to the clinic.  Please arrive 10 minutes before your appointment time. 

 

It is recommended to bring your most commonly worn pair of shoes, and orthotics that may have been prescribed to you in the past.

 

If you have difficulty printing the New Patient Form, please arrive 20 minutes before your appointment to fill out the intake form at the clinic. Please be prepared with all necessary information, including a list of medications.
